The ASQ:SE system includes

  • 8 questionnaires and corresponding scoring sheets (on CD-ROM and paper—now included in both formats in every ASQ:SE box!). Parents complete the short, simple questionnaires at eight designated intervals: 6, 12, 18, 24, 30, 36, 48, and 60 months. Set up like the ASQ-3, these questionnaires prompt parents to answer 22–36 questions with most of the time, sometimes, and rarely or never. Each questionnaire takes just 10–15 minutes for parents to complete and addresses seven crucial behavioral areas: self-regulation, compliance, communication, adaptive functioning, autonomy, affect, and interaction with people.

    For every questionnaire, there’s an easy-to-use form that allows professionals to score the test in 1-3 minutes and determine the child’s progress in that social-emotional area. The reproducible scoring sheets all include referral considerations that help determine if the child needs further evaluation.

  • The ASQ:SE User’s Guide. Clear and comprehensive, the User’s Guide walks professionals step-by-step through the process of setting up and running ASQ:SE. The User’s Guide provides instructions on administering the questionnaires with sensitivity to children’s environmental, cultural, and social-emotional differences, detailed technical data on how the system was developed and tested, helpful case examples, and creative activities and lists of social-emotional behaviors professionals can share with parents for use with each age group. Spanish translations of letters to parents and selected forms are also included.

The ASQ:SE Starter Kit includes 8 color-coded, reproducible questionnaires, now on CD-ROM and paper for use at 6, 12, 18, 24, 30, 36, 48, and 60 months of age; 8 reproducible scoring sheets—1 for each questionnaire; and the User’s Guide.
